Makeup Consent

Mobilehouseparty.com, requires you the Parent to print and sign this waiver. As part of the Spa experience, age appropriate makeup and spa treatment products will be applied. This includes, but not limited to lip gloss and eye shadow. Makeup products used are hypoallergenic and makeup applicators are new and disposable. I acknowledge that any of the beauty or “spa treatments are for entertainment purposes only and Mobilehouseparty.com are not licensed cosmetology professionals. Mobilehouseparty.com will not cut, file, trim, push back cuticles or do any services provided at a certified nail salon. Mobilehouseparty.com is an event planning company and the spa party themes are for entertainment only.

Photo Release Form for Minors
I give Mobilehouseparty.com, permission to use my child’s likeness, image, voice, and/ or appearances as such that may be embodied in any pictures, photos, video recordings, digital images, and the like, taken or made on behalf of Mobilehouseparty.com, party activities. I agree That Mobilehouseparty.com has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Mobilehouseparty.com Parties. These uses include, but are not limited to, reprints, publications, advertisements, social media and any promotional materials in any medium now known or later developed, including the internet. I acknowledge that I will not receive any compensation, etc for the use of such pictures, etc., and hereby release Mobilehouseparty.com, from any and all claims which are in any way connected with such use. I have read and understood this consent and release.

Food Allergy Waiver
Your child will be participating in an entertaining spa like activities. Some of these activities involve food products as part of the experience. We know that many children have allergies and some of these allergies can be life threatening. We take many precautions to ensure that all the children are safe. Mobilehouseparty.com will not be held responsible for any allergic reaction or life-threatening incident including death that may occur. If your child has any known food allergies or intolerances that will prevent her from participating in these activities, we will provide an alternate option at your request.

LIABILITY
We Strictly provide entertainment. We cannot be held responsible for accidental damage caused by your child. Mobilehouseparty.com, assumes no responsibility or liability for accidents or damage caused during the party. In the event of damage or loss or our inventory and/or damage or loss to the party bus, the responsibility lies entirely on the client and or guardian. Mobilehouseparty.com, is not responsible for allergic reactions and / or any medical injuries from the food, beverages, wearing costumes or any activity taking place before, during or after the party. I hereby release and agree to hold . Mobilehouseparty.com harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or that may otherwise arise in any way in connection with any services received from .
